Central City Concern (CCC) is a nonprofit, Portland-based FQHC dedicated to providing comprehensive solutions for individuals experiencing homelessness, poverty, and addiction. With integrated services spanning housing, employment, and healthcare, CCC serves thousands of vulnerable individuals annually. As the organization expanded its programs and adopted new digital systems—including Epic for healthcare and Apricot for Supportive Housing case management—it found itself facing a growing gap between data complexity and actionable insight.

To bridge that gap, CCC partnered with Skypoint to build a modern data platform capable of supporting advanced analytics, cross-program coordination, and long-term program design—all while democratizing access to data across the organization.

The Challenge: Fragmented Data and Rising Demands

CCC’s existing data warehouse, originally built more than a decade ago, was not designed to accommodate new systems or modern analytics demands. As CCC implemented Epic to replace its aging EHR and introduced Apricot to centralize case management data, the limitations of the legacy system quickly became apparent.

Long-running data jobs, inconsistent data definitions across systems, and manual report generation slowed down operations and frustrated both IT staff and business leaders. CCC needed a flexible, scalable solution that could not only accommodate new data sources but also support citizen analytics and outcome measurement.

We had three things going on at the same time: new data, new systems with different data, and an increased need for reporting. Our current data warehouse just wasn’t suited for all of these changes.

When Data Governance Becomes Everyone’s Job

As new systems came online, CCC’s teams began to realize that the real challenge wasn’t just technical—it was also organizational. Program definitions, data quality, and engagement metrics varied significantly across departments. Without a shared understanding of what data meant, it was difficult to draw meaningful insights or measure the effectiveness of CCC’s holistic service model.

One key area of focus became defining “program enrollment” consistently across health services, behavioral health, housing, and employment, so that CCC could begin to ask critical questions: What combinations of services lead to better outcomes? How does access to housing influence healthcare costs?

The main impact so far has been a broadening realization of the importance of data governance… The workflows and tools we use to collect data directly impact what we can derive from it.

A New Platform for Cross-Functional Insight

Skypoint is helping CCC modernize its data environment through a multi-phase Enterprise Data Warehouse (EDW) revitalization project. The new platform consolidates data from Epic, Apricot, and historical systems while enabling self-service analytics. This will empower CCC’s business units to generate their own reports and surface actionable insights without relying solely on the IT team.

This new infrastructure lays the foundation for CCC’s long-term vision: to enable AI-driven analytics, reduce administrative burden, and improve service delivery by understanding the full journey of the individuals they serve.

Our overall strategy is to simplify our data ecosystem and upgrade our technology from an on-prem paradigm to a hybrid or cloud model. We’re trying to make it easier for our end users to leverage and access all the data that CCC is uniquely positioned to capture.

Our business partners who have been involved are ecstatic about what’s available now, compared to what they couldn’t do before. It’s a success story that we can build on as we grow and expand into other areas

A Vision for the Future: Equity, Efficiency, and Insight

CCC is already seeing the benefits of more integrated data in specialized programs like Recuperative Care, where housing, healthcare, and case management are offered in a fully coordinated model. Data from this program has revealed powerful cost and utilization outcomes, which CCC hopes to replicate at scale.

By enabling outcome measurement and smarter service design, Skypoint’s platform is helping CCC move from reactive reporting to proactive program strategy. It’s not just a technical upgrade—it’s a reimagining of how data can drive human impact.
